Social Worker/Case Manager #17-49
CMP Part-Time, Oahu
The Social Worker/Case Manager provides professional counseling and case management services for clients of assigned elderly population. Position involves evaluating and assessing potential clients’ needs, linking them to appropriate services and/or assisting with placement as needed. Employee must be also able to travel to prospective clients’ and referral sources sites, conduct regular office/home visits and visits to vendors’ business locations. The Social Worker/Case Manager must demonstrate an active commitment to Catholic Charities Mission and strive to incorporate the four Core Values in all aspects of daily work.
EDUCATION & WORK REQUIREMENTS: Must have a Master’s degree in Social Work from an accredited university plus a minimum of three (3) years experience working with the elderly population. Must be able to obtain a LSW within the first year of employment. In-person interview required.
EMPLOYMENT CLEARANCE REQUIREMENTS: Applicable background checks; HI State criminal check, TB clearance, valid driver’s license, copy of no-fault automobile insurance, access to insured vehicle, traffic abstract, and CPR/First Aid.
