Horizons Independent Living Program

Providing young adults a pathway to stability and independence

The Horizons Independent Living Program supports young adults ages 17–24 as they navigate the critical transition from adolescence to adulthood. Designed especially for former foster youth and young people exiting the juvenile justice or mental health systems, Horizons offers a safe and supportive home environment where residents can grow in independence, confidence, and life readiness.

 

Why Horizons Matters

  • 60–90 youth exit foster care annually (2021–2022)
  • 217 homeless youth (ages 18–24) identified in the 2024 O‘ahu PIT Count
  • Street Youth Study findings:
    • 42% previously in foster care
    • 45% involved in juvenile detention
    • 38% experiencing mental illness

 

Horizons 1 – Salt Lake, O‘ahu

Who It Serves
Young adults ages 17–24 transitioning from foster care, juvenile justice, or mental health systems.

Program Focus
Short-term, highly structured transitional housing focused on building foundational independent living skills.

Support Provided
• Intensive case management
• Dedicated Transition Facilitator
• Life skills training (budgeting, cooking, employment, transportation, education)
• Short-term counseling and referrals

Length of Stay / Timeline
Youth typically transition to Horizons 2 or another independent living option within 9 months.

Financial Component
Optional matched savings program: youth savings matched by CCH up to $400, returned upon discharge.

Primary Goals
• Prepare for independent living
• Increase employment and education participation
• Prevent homelessness

Horizons 2 – Makiki, O‘ahu

Who It Serves
Young adults ages 18–24 who are relatively independent but need affordable, supportive housing.

Program Focus
Long-term independence and self-sufficiency through permanent supportive housing.

Support Provided
• Resident-driven support
• Transition Facilitator available for general guidance
• Optional life skills coaching and therapy upon request

Length of Stay / Timeline
No fixed time limit; residents may stay until age 24.

Housing Costs
• $500/month – Single room, shared bath
• $600/month – Primary room with private bath

Primary Goals
• Maintain housing stability
• Encourage self-directed growth
• Support long-term independence
